If your account has been locked or you have forgotten your password, you can have it reset from this page. In order to proceed, you must have access to a telephone with a direct dial number or can receive text messages. After providing the information below, our system will call you at the telephone number you enter below or send you a text message to provide you with a reset code. You must enter that code on the next page along with your new password to have it reset.

Your Participant ID is a unique ID number that was assigned to you when you initially enrolled in your account with Flores. This was sent to you by mail, or email, if an email address was provided at the time of your enrollment.

Your PID can also be found on rejection notices, reimbursement checks, reimbursement confirmation emails, claim forms, and other email notifications you may have received from Flores in the past.

If you are unable to locate this number, your HR department may be able to assist you, or you may also contact Flores at 800-840-7684 during our normal business hours - 8:30 am to 5:00 pm EST.

This process requires you to have received a Flores Debit Card. If your Plan does not use a debit card, you should expect an email or letter with your Participant ID number and instructions to access the website.
